Home care in Itahari is often planned in the wrong city. Itahari, in Sunsari, sits on the roads that run toward Dharan one way and Biratnagar the other. Families say "the east" and then give a nurse an address that is a different municipality. Dharan has a large teaching hospital. That does not make a house in Itahari a Dharan house, and it does not mean the hospital sends a nurse home with you. Biratnagar is a separate metropolitan city again. Ask which problems mean you return to that hospital and which problems should go to the nearest emergency department tonight. Those are different answers. Write both.
Do not change a prescription on the way because a pharmacy in Itahari offers a similar box. If the prescribed item is unavailable, call the prescribing team before you swap it.
The person who will do the nights may not have been at the hospital. Sit them down the same day. The World Health Organization's ageing guidance assumes families do a large share of daily support. That only works if the family member in the house has heard the danger signs.
